When you come into care, we make plans with you. We listen to what you say and to the people who know you best. Our plans support your:
- relationships
- learning
- health
- hobbies
- home
- mental health
Your plans
Sometimes you only need to be in care for a short time before you return to your family. Other times, it can be longer or permanent.
To help us decide the best plan for you and your family, we will do an assessment. The assessment means we talk about what support you need, listen to your views and your parents' or carers views, and make a plan for your care.
Care plan
A care plan explains how we will support your:

Placement plan
The placement plan explains:
- where and with whom you will live
- any special arrangements such as travel to school
