Support for people living with dementia

Social care support for people living with dementia

If you are living with dementia, we work with our partners to ensure you have access to relevant information, advice and guidance. We may also be able to help arrange support with things such as: 

  • eating and drinking
  • keeping yourself clean
  • going to the toilet
  • being appropriately clothed
  • keeping safe in your home
  • keeping your home clean and tidy
  • seeing your family and friends
  • finding and attending work, training, education, or volunteering
  • making use of necessary facilities or services in the local community including public transport, and recreational facilities or services
  • carrying out any caring responsibilities you have for a child

Integrated mental health services

Some people living with dementia have very complex needs. To provide the right support, we work in partnership with Avon and Wiltshire Mental Health Partnership NHS Trust (AWP).

Some of our adult social care workers are based in the Complex Intervention and Treatment (CIT)Team. 

They work alongside

  • psychiatrists
  • psychologists
  • occupational therapists
  • psychiatric nurses

to ensure that health and social care needs are considered together.

To access support from this team, you will usually need to be referred by a health professional, such as your GP.  

Your GP can also refer you to AWP’s memory services in North Somerset. They offer assessment, diagnosis and treatment for people experiencing difficulties with their memory and thinking skills. These difficulties may be affecting your everyday life.

Day services for people with dementia

There are a range of day services available for people with dementia throughout North Somerset. They can help adults with dementia keep doing things for themselves and offer those caring for them a break from their caring role. 

Dementia wrap around care

The Dementia Wrap Around Care Team (DWACT) is a 24/7 urgent service that provides wrap around home care for people living with dementia and their carers.

It aims is to avoid crisis situation such as hospital admission /re-admission or care home placement.

Dementia care assistance

If you or someone you care for has been diagnosed with dementia, further grant aid for minor adaptations may also be available.
