How to request a carer's assessment

We base a carer’s eligibility for an assessment only on the ‘appearance of need’ for care and support.  Under the Care Act 2014, we must offer an assessment on this basis. 

The carer’s assessment lets you discuss

  • your caring role
  • the impact it may have on your health and well-being
  • what support would be of benefit to you

Under the Care Act 2014, carers have entitlements to care and support in their own right. They can be eligible for support, even if the person they care for has refused support or is not eligible.

How to request a carer's assessment - adult carers

If you are caring for a person who lives in North Somerset, you can request a carer’s assessment by: 

  • filling in our online self referral form
  • phoning Care Connect on 01275 888 801 

If the person you are caring for lives in another local authority area, you should contact their local council for help and support. 

Changes to your assessment

If you have had a carer’s assessment with us and your circumstances or needs have changed since, you can request a review of your carer’s assessment.
