The i-Thrive framework

Most children and young people thrive with the support of family, friends and other important adults in their lives. Sirona Children’s Services are here for you and your family if, and when, you need a little more help.

We will work with you and other services to look at what options are available to support you and your family. The support available is based on five different needs-based groupings:

  • information and signposting - this means “I need someone to help me find out where to go for advice.”
  • getting help - this means “I need help from a trained person. We set goals and have a plan in place if things do not help."
  • getting more help - this means “I need more help from trained people in different services. We set goals and have a plan in place if things do not help.”
  • getting risk support - this means “Sometimes the help that people are offering me is not working which is having a significant impact on my well-being and safety. Those around me are working together to keep me safe.”
  • thriving - this means “I know how to keep myself healthy and well. I know where to get support to do that.”

Access to GPs and annual health checks

Annual health checks are for adults and young people aged 14 or over with a learning disability.

An annual health check helps you stay well by talking about your health and finding any problems early so you get the right care.
