How we define eligible

In some situations, we have a legal duty to help people with care and support needs. For us to have this duty, your care and support needs must arise from a physical or mental impairment or illness. 

This might include

  • a physical disability
  • learning disability
  • mental ill health
  • a condition such as autism.

Before we can provide support, we follow a process to decide if you have eligible care and support needs. This is called a care assessment.  

How we decide eligibility

We will first have a conversation with you or someone else you nominate to speak with us. We will ask about your care and support needs and the goals you want to achieve. We may be able to provide you with information and advice which can help you achieve your goals.   

If our information, advice and guidance does not support you to achieve your goals, we will arrange a care assessment under the Care Act 2014. 

Not everyone will need a care assessment, and not everyone who is assessed will be eligible for care and support.   

A care assessment involves a trained social care practitioner, such as a social worker, talking to you about your care and support needs. Depending on your requirements, this could be over the phone, online or in-person. "

If you do not agree with our decision around eligibility

If you think an important piece of information was not considered or that your circumstances have changed, first contact Care Connect. Details are included on this page.

Someone will then decide whether it is appropriate to reconsider the original decision or undertake a further needs assessment. Sometimes, the decision will still be that you are ineligible for support.
